What is a Primary Care Network?
The UK population is larger and living longer than ever before with many of us now experiencing conditions that are both longer term and that require additional access to healthcare services. To meet these increasing needs, practices have begun to work together in Primary Care Networks (PCN’s). By collaborating at scale with our community, mental health, social care, pharmacy and hospital services, as well as the voluntary sector, we can deliver better patient outcomes so that we can all live happier and healthier.
Smallfield Surgery, along with Birchwood Medical Practice and Wayside Practices, make up your local PCN, called Healthy Horley.

What this means for you
By collaborating as a PCN we are improving the health of our patients. We are working together to evolve our traditional reactive systems for services that are based on appointments to the provision of social and health care that is personalised and individual to you, the patient.
This brings greater access to a wider range of coordinated health and social care professionals working as part of community based teams. These include nurses, therapists, pharmacists, social prescribers and care providers.
In turn, our GP’s are able to focus more on patients with complex clinical needs as well as those with longer term conditions. With the support of our community teams we are able to deliver care that is tailored to need and accessible in the community, helping you to prevent ill health and allowing happier, healthier living for us all
