If you live in our practice area (see the map on the next tab) and would like to register with us, please complete the registration forms that are available from reception, or below to print and return them to reception. When you bring in your registration forms we will ask you to provide a photo ID and proof of residence if possible. If you do not have this, providing you are going to be residing in our practice area for a minimum of three months, we should be able to register you. Patients are registering with the practice rather than an individual GP. You will however be allocated a named GP and you will be informed of this following your registration. You are welcome to specify a preferred doctor if you wish.
Please ensure that all the forms are printed and completed in full and then bring them into reception with all the necessary indentification documents as we do not accept completed forms by EMAIL or POST- Thank you
Once you have registered we will encourage you to book a new patient check which is essential if you are on any repeat medication. Please bring your repeat slip from your previous surgery with you.
