Patient Registration

filesIf you live in our practice area (see the map on the next tab) and would like to register with us, please complete one of the registration forms that are available from reception, or print one out below.  When you bring in your registration forms we will ask for proof of identity and proof of address.  You will be registering with the practice rather than an individual GP, but you are welcome to specify a preferred doctor if you wish. 

You can print out a new patient registration form below and you need to fill it out and bring it in with you on your first visit to the practice.

Medical questionnaire

We will also ask you to complete a form that gives us some basic information about your medical history and you will be encouraged to book for a basic check with one of our nurses.  Such a check is essential if you are on repeat medication. 

You can print out the form below, so that you can complete it before attending the surgery to complete the registration process.

Measles Alert

If your child has not been fully immunised against measles, please contact the surgery as soon as possible to book an appointment in our special clinic on Friday 21st June in the afternoon.

Electronic Prescription Service 

You can now have your prescriptions sent electronically to your preferred pharmacy.  All you have to do is nominate your pharmacy of choice with reception.

Online Services

Did you know you can book appointments request repeat prescriptions online?.  To make use of these services you first need to reigster with us for Vision Online Services (VOS).  You can pick up the necessary form in the surgery, or via this link VOS registration form

The completed form needs to be returned to the surgery.

Patient Group

We now have a Patient Reference Group to enable us to obtain views from a sample of our patients about current and future services.  If you would like to join this group, which will involve giving us you email address, or another form of contact, and some other details, please click the appropriate  option  in the list below.
