Patient requests for letters

" Unfortunately pressures on General Practice mean that we are having to review our work to ensure that we are able to focus on our key duty of care for patients. We are therefore unable to provide the following until further notice:

We can no longer provide character references, countersigning documents (visas, passports) or confirmation of identity and/or witness to signature, when this can be done by another non-medical professional.

Requests for letters or reports regarding re-housing and "Blue Badge" applications and requests to supply patients with letters in support of benefit appeals beyond normal statutory processes to request further medical information. (Letters of these kinds are an expense for the patient and the kind of information contained often doesn't carry weight, but takes up inappropriate GP and staff time" - Taken form the BMA website

Ordering Prescriptions On-Line

If you have previously notified us of a pharmacy for your prescriptions you DO NOT need to fill in the pharmacy again with each request. This will help speed up the process for our receptionists. Thank you

Change of Medical System

On the 5th November we successfully changed our Medical System from Vision to EMIS Web. Work is still going on behind the scenes as we get used to the new system, so do still bear with us.  Our On-Line access is now available so please either ask at reception or ring the surgery to request your new registration letter which will allow you to create an account on the new system. Thank you

Flu Clinics - 2016

Appointments are available for this years flu vaccination campaign. If you are under 65 years of age and in an "at risk" category or are over 65 years then please contact the surgery on 01342 843822 to book an appointment. We also have vaccinations available for children which can be booked in the same way. Please ask at Reception for details of all the clinics.


Mjog Text messaging Service

We now offer a text messaging service. This means that if we have your mobile phone number you will receive confirmation of any appointment booked at the surgery. You will then receive a reminder prior to the appointment. We may also contact you in this way to invite you to come in for an appointment e.g for a flu vaccination or a health check. If you do not to be contacted in this way then please let reception know.

All telephone calls will now be recorded. This is for training purposes only

With effect from 31st March you now have access to part of your Medical Records. You will need to complete our most up to date on-line application form. For more information about this service please ask at Reception.

Saturday Opening

The surgery is open every Saturday from 09.00 until 12.00  for GP Appointments. Some of these can be pre-booked with the remainder being available on the day for something which cannot wait until monday.  Nursing appointments are available on alternate Saturdays and can be pre-booked. Please call for an appointment. 

Supporting people with memory problems 

Every week there is a community meeting in the Centenary Hall. If you care for someone who has memory problems we would be pleased to see you. Please let reception know if you are interested. We also need helpers. 


Online Services


We offer on-line booking of appointments and online requesting of repeat prescriptions.  To make use of these services you first need to reigster with us for Patient services. you can either click on the link below or collect a form from reception. Please complete the form and return it in person to reception along with your identity documents as indicated below.

 Patient Access On-Line Registration Form

When registering for Patient Services you can have access to the following:

  • Make and cancel appointments
  • Request Repeat Medication

 However, from March 2015 you will also have access to the following:

 Access to your Medical Records – this will enable you to view your Medical records on-line.

 In order to do this we are now required to confirm your identity and record the confirmation in your medical records. This is an NHS prerequisite for any patient to access their medical records.

 Two forms of documentation must be provided as evidence of identity and one of these must contain a photo. Acceptable documents include passports, photo driving licences and bank statements, but not bills.

  For all new patients this will be part of the Registration Process but for existing on-line users we will need to confirm your identity.

 In view of this would our existing users kindly supply us with the recommended identification to allow us to record it in your medical records?

 The completed form needs to be returned to the surgery and you will then be issued with the details you need to create your own username and password on the website.
